The home sleep test: one fewer night away
For many adults with snoring, witnessed pauses in breathing and daytime sleepiness, a home sleep apnea test is enough to diagnose obstructive sleep apnea. After your consultation you take a small device home, wear it for the night in your own bed, and bring it back. There is no evening drive to the lab and no morning drive home after a night away. The trade-offs are covered in in-lab vs. home sleep study.
What still needs a night in the lab
An overnight in-lab study is the right test when:
- you have significant heart or lung disease, or central sleep apnea is suspected;
- the question is narcolepsy, which also needs daytime nap testing (MSLT) the next day, or wakefulness testing (MWT); see MSLT and MWT;
- the patient is a child: pediatric sleep studies are done in the lab;
- CPAP or BiPAP pressure needs to be set in a titration study;
- a home test was negative but sleep apnea is still likely.
For a lab night, plan to arrive in the evening and leave in the morning. Bring what you need to sleep and to get ready for the day. If you are very sleepy, arrange for someone else to drive you home. What the night involves: what happens during a sleep study.
Treatment and follow-up with less driving
- CPAP and BiPAP: the machine records how it is working, and many check-ins can be done by telemedicine. CPAP therapy.
- Insomnia: CBT-I is a series of sessions on sleep habits and schedules, and some can be done by telemedicine.
- If CPAP doesn't work for you: an Inspire evaluation and other options.
